Alternative to Transfer Pilot Scheme
gtd healthcare are working with the North West Ambulance Service in providing an alternative to transfer to hospital for patients who have called an ambulance, but would benefit from seeing a GP working within the community.
The Pathfinder tool identifies patients who may be suitable for referral and so far, over 90% of patients referred to the scheme, are cared for in the community.
The scheme supports the ‘Care Closer to Home’ and Urgent Care Strategy agenda which allows patients to be cared for safely in the community, rather than having to attend hospital. The service links into other community services to ensure a multi-disciplinary, multi-agency approach to patient care.
Since October 2013, the pilot scheme has covered the areas of Central Manchester, South Manchester, Tameside & Glossop and Oldham. North Manchester commissioned the service from June 2014.
Feedback from the scheme has been extremely positive from both patients and paramedics, below are some examples of the feedback received to date.
