An exciting opportunity has arisen for specialist palliative care clinicians to join a newly established service, which has been developed to provide patients and their families with seamless care out-of-hours.

We are looking for expert practitioners who are not only experienced specialist palliative care clinicians, but are highly empathetic and motivated to drive forward a service that improves the quality of care palliative patients receive in the out-of-hours period.

You will have a passion, commitment and determination to ensure continual service improvement and you will be eager to make a difference. Working as part of a multidisciplinary team, the successful candidates will support patients, their carers/relatives during the out-of-hours period including undertaking home visits and providing telephone advice. gtd healthcare recognises the importance of investing in a team of specialist palliative care clinicians and realise their potential as part of the organisation’s multi-disciplinary team model.

You must have the following;

·Registered first level nurse, paramedic or other AHP.

·Be a recognised Specialist Palliative Care Clinician

·1st Level Degree

·Excellent standards of care and a positive attitude to managing change and developing service

·Excellent communication skills, both written and verbal

·Ability to communicate and work collaboratively within a wide range of disciplines
