In the meantime, you can access the Manchester Urgent Primary Care Hub whilst your registration is being processed.
If you require urgent but not life-threatening medical assistance that cannot wait until you are registered, please call 111. Following a telephone assessment by NHS 111 you will be provided with information about the appropriate service for your health problem. If you are unsure which health care service you need to visit, NHS 111 will signpost you to the most appropriate care for your condition - GP, local pharmacy, emergency department or an emergency ambulance response.

M1, M2, M3, M4 and M15 IF IN CASTLEFIELD. IF YOUR POSTCODE FALLS OUTSIDE OF THIS, PLEASE DO NOT ATTEMPT TO REGISTER AS IT WILL BE REJECTED.

PLEASE NOTE: TO REGISTER, YOU MUST DOWNLOAD THE REGISTRATION FORM AND SAVE THE FORM TO YOUR DESKTOP FIRST BEFORE COMPLETING ANY OF THE INFORMATION.

The form should only take a few minutes of your time to complete.

Once you have completed all areas of the form, please:

  • re-save the form on to your desktop.
  • go back to the ‘Register online’ page on the website and upload the completed patient registration form.
  • click submit (after verifying you have read the privacy policy).
  • once submitted, the form will be sent to the GP practice for processing.

Please do not include personally identifiable information when submitting the patient registration form.
