To update details such as your name, phone number or email address:
- fill out our online request form;
- login to your NHS account;
- phone or visit the practice Monday to Friday, from 10am to 6pm.
Please note:
- If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice. Find out which GPs you can register with on the NHS website.
- If you have changed your name you will need to show us proof. This could be a marriage or civil partnership certificate or deed poll. Please bring this proof into the practice so we can view it.
