Changing your contact details
To update your phone number or email address, please login to your NHS account.
NHS app:
- Click the person icon in the top right corner.
- Under settings, click contact and login details.
- Click contact details.
NHS account (web version):
- Login to your NHS account.
- Click account in the top right.
- Under settings, click contact and login details.
- Click contact details.
Alternatively, you can phone or visit the practice. Please note, all calls are recorded for training and quality purposes.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice.
Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
