Changing your contact details

To update your phone number or email address, please login to your NHS account.

NHS app:

  • Click the person icon in the top right corner.
  • Under settings, click contact and login details.
  • Click contact details.

NHS account (web version):

  • Login to your NHS account.
  • Click account in the top right.
  • Under settings, click contact and login details.
  • Click contact details.

Alternatively, you can phone or visit the practice. Please note, all calls are recorded for training and quality purposes.

Change of address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice.

Changing your date-of-birth or name

You will need to show us proof that you have changed your name or date-of-birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
