Changing your contact details

To update your phone number or email address, please phone or visit the practice. Please note, all calls are recorded for training and quality purposes.

Change of address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice.

Our catchment area: M1, M2, M3, M4 and M15, if in Castlefield. Patients who live further away may be able to be registered but would need to speak to a member of the reception team and home visits would not be available.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
