If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice.
Our catchment area: M1, M2, M3, M4 and M15, if in Castlefield. Patients who live further away may be able to be registered but would need to speak to a member of the reception team and home visits would not be available.
You will need to show us identification before we can change your phone number or email address.
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
