Before filling out a registration form, check you are in our catchment area.

Registering with the practice

To register with the practice:

  • fill out an online registration form (see further guidance below);
  • visit reception and collect a paper registration form.

When you register, it is helpful to have your NHS number. You can use the NHS website to find your NHS number.

Online registration form

Please complete a new patient registration form for each member of the family that you wish to register including children.

  1. Download the registration form and save it on your desktop before completing any of the information.
  2. Complete all areas of the form.
  3. Re-save the form onto your desktop.
  4. Go back to the Join the practice page on the website and upload the completed patient registration form (see below).
  5. Click submit (after verifying you have read the privacy policy).

It can take up to 10 working days for a patient to be registered from the date it is submitted. Once you are registered we will invite you for a patient health check. If you have any queries, please contact the reception team.

Required
Named accountable GP

All registered patients are allocated an accountable GP. The accountable GP takes responsibility for the co-ordination of all medical services and ensures they are delivered to each of their patients where required. This is to reassure patients that they have one GP within the practice who is responsible for ensuring that work is carried out on their behalf and for continuity of care.

You can access the Manchester Urgent Primary Care Hub while your registration is being processed.

If you require urgent but not life-threatening medical assistance that cannot wait until you are registered, please call 111. Following a telephone assessment by NHS 111 you will be provided with information about the appropriate service for your health problem. If you are unsure which health care service you need to visit, NHS 111 will signpost you to the most appropriate care for your condition - GP, local pharmacy, emergency department or an emergency ambulance response.
